Please ensure that the following documents are completed and submitted to the Manager who is consulting with you for onward transmission to the Screening Department:

1. TUPE Transfer Form
2. TUPE Loan Request Form (if applicable)
3. Working Time Regulation Statement Form
4. Holiday Request Form
5. Uniform Request Form
6. Official Identification documents
   
   1. Birth Certificate / Passport
   2. Proof of Right to work (if applicable)
   3. Proof of National Insurance Number
   4. Proof of Address (not older than 3 months)

It is essential that we receive the above documentation as soon as possible in order that we can satisfactorily facilitate your transfer to Lodge service.
